I do hereby waive, release, and forever discharge Axis Pilates and The Pilates Architects (further named The Company) any associated businesses, facilities, where classes and sessions are held, from any and all responsibilities or liability from injuries or damages, death or dismemberment resulting from my participation in any and all events.
I understand that physical activity may involve risk of injury and even death, and I am voluntarily participating in these activities and using equipment and machinery with knowledge of the dangers involved. I expressly assume and accept all risks.
I hereby release and agree to hold all parties associated with The Company, and the facility where classes and sessions are held, harmless from any causes of action, claims, demands, damages, costs, expenses and compensation for damage to myself that may be caused by any act, or failure to act, or that may otherwise arise in any way with any services received.
I understand that this release discharges the aforementioned from any liability with respect to bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received.
This liability waiver and release extends to all owners, partners, and employees.

Media Release

All pictures, videos, and recordings are the property of The Company. They may be used for any publication including, but not limited to; websites, print, and social media.

Allowing the use of media is super helpful for the business and greatly appreciated! However, if for any reason you do not feel comfortable with this we understand and respect your privacy.
